Fibromyalgia (FM) is a chronic, generalized and diffuse pain disorder accompanied by symptoms such as fatigue, sleep disturbance, depression, anxiety and cognitive deficits. The etiology and pathophysiology of FM are widely unknown, and there are no specific somatic signs of the disease. The currently most common view is that FM pain results from hyper-activity of central nociceptive pathways and deficient pain inhibition. Although FM patients´ complaints about cognitive impairments are very frequent, few studies have comprehensively investigated their neuropsychological functioning. In addition, relationships between the cognitive deficits and autonomic regulation, cerebral blood flow and central nervous sensitization to pain have not been studied so far. In the project we will accomplish a neuropsychological evaluation in FM using a comprehensive set of tasks. As possible relevant factors for the deficits we will take into account a) clinical pain, b) psychiatric co-morbidity, c) emotional states, d) fatigue, e) sleep disturbances, f) medication use, g) autonomic cardiovascular parameters, e.g. blood pressure and baroreflex sensitivity, h) cerebral blood flow responses to cognitive challengers assessed by functional transcranial Doppler sonography, and i) central nervous sensitization to pain.
